The previous version had a link to http://creativecommons.org/about/licenses in
the middle column that's somewhat useful.  Let's link the word "license" in the
second intro paragraph to that.

The form looks a little wide now; add CSS to constrain the gray boxed form to be
the same width it was before.

Removed the bar, and added the language drop down list.

Note: the javascript for the language SELECT will cause the page to lose any GET
params that were previously there, does the license engine index page utilize
any other GET params?

The introductory text on http://creativecommons.org/license should clearly
explain that users are selecting a license, not generating one.
